After you have completed this information, our pre-surgical nurse will review the information entered which will assist in documenting your complete medical history in preparation for the procedure. Please be assured that all of your information is confidential.
Before you begin One Medical Passport, please have the following information ready to streamline the process:
- Your health insurance details
- Names, addresses and phone numbers of your physicians
- All the medications you are taking, dosages and frequency
- A list of all the surgical procedures you have had
- The name of the surgeon who will be performing your procedure
About One Medical Passport
Completing your medical history online with One Medical Passport is easy. For most patients, completing their entire medical history takes about 20 to 25 minutes. At any point, you can stop completing the questionnaire and come back later to finish the remaining sections.
The information will be thoroughly reviewed by your medical team. By filling out the history questions accurately and completely, it will assist and streamline the preparations for your surgery. It will be thoroughly reviewed by your medical team.
With One Medical Passport, you can enter your information from anywhere. You can also print a copy of your history after you entered it and will have access to it online anytime for future use or to update, as needed.
